Requesting a new shipping label is only applicable to those who are submitting a DS-82 form for renewal and who have purchased a shipping label from us. Keep in mind that only those who are eligible for renewal can submit their applications through the mail. Reporting a lost or stolen passport is only possible for those who don't need a new one. However, we don't provide a shipping label for DS-64 or for reports of lost or stolen passports - those documents should be mailed out at the customer's expense.
There are three occasions when a new shipping label may be requested:
1. When a customer's renewal application is returned for any reason or the shipping label has expired, the option to request a new shipping label is included as part of our customer service fees.
2. When a customer requests changes to their passport form:
- First and Last Name
- Primary Phone Number
- Mailing Address - Ensure that a permanent address is provided on the form if the mailing address provided is a Post Office (PO) box.
3. When a customer alters their processing time from routine to expedited, or vice versa.
To request a new shipping label, follow these steps:
1. Verify if the customer is eligible to request a new shipping label by referring to the image provided.
- To verify if a customer has mailed out their application, one can use either Google to track the USPS tracking number or ask the customer directly.
- If customer mailed out their application already, direct them to call National Passport Information Center at 1-877-487-2778 or 1-888-874-7793 (TDD/TTY).
2. Confirm if the customer has purchased a shipping label from us previously by checking the order app and the "Type" and "USPS Tracking Number" sections.
